Abstract

The surgical techniques of aortic valve-sparing operations emerge in the early 90s, as an alternative to the standard treatment of valve and ascending aorta replacement for those patients with aortic root dilatation with or without aortic valvular insufficiency. The valve preservation, avoiding the use of valve prostheses and anticoagulant therapy (especially in young patients or in the context of collagen diseases as Marfan syndrome) are the main advantages provided by these techniques, becoming the preferred surgical procedure to treat aortic root aneurysm in those patients, although its implementation involves more complex technique. There are basically two main techniques of aortic valve-sparing operations: the root remodeling or Yacoub technique, and the aortic valve reimplantation or David Technique. Both have shown acceptable short and long-term results. Herein, it is described a serie of 80 patients submitted to aortic valve-sparing operations in the Cardiovascular Surgery Department of the University Hospital Marqués de Valdecilla, in the period between 1 January 1994 and June 31, 2015. This is a prospective and protocolized analysis, describing the serie with and analysis in terms of survival, early and long-term surgical outcomes and predictors of survival, stability and valve reoperation.
