Resumo

Introduction: Epilepsy is a neurological disorder that presents with recurrent seizures. Although the treatment of choice is pharmacological, there are cases of ineffective response to it. In refractory epilepsy with an ineffective response to pharmacological treatment, the ketogenic diet can be considered as adjunctive treatment. The ketogenic diet is characterized by providing most of the caloric percentage through lipids, which implies an alteration of the physiological metabolism depending on the energy needs of the individual. Method: In this article the evaluation of the existing bibliography regarding the effectiveness of the ketogenic diet as part of the therapeutic approach to refractory epilepsy is proposed. Discussion: There is heterogeneity in the consulted scientific literature. However, the authors agree that the ketogenic diet implies a change in the source of energy used for metabolism, which is mostly fat. The use of fats as a source of metabolic energy can lead to adverse effects for the body, so it must be individualized and controlled over time, according to the energy needs required by the individual. Conclusion: Although the scientific literature does not reflect strong scientific evidence in this regard, the ketogenic diet could be considered as a feasible option for refractory epilepsy as an adjunctive treatment for remission of seizure status.
